Clinical and statistical study of gastric cancer


Abstract
The cases of 94 patients recieved operation at the Dept. of surgery, Kangnarn municipal hospital between 1st, Oct., 1976 and 31th, Sept., 1980 with gastric cancer were studied statistically, and following results were obtained.
1) Sex ratio was 1.9: 1 male to female, and most frequent age group of gastric cancer was from 50 to 59 years.
2) The most common symptoms were epigastric pain 72.34%, epigastric tenderness includ mild cases 77.65% weight loss 52.12% palpable mass in the epigastric area 48.93%, indigestion 45.74% and nausea and vomiting 38.29%.
3) Duration of symptoms to operation were predominate in the group of 3-6 months: with 39.36%, less than 3 months with 18.08% and in the group of more than 1 year with 28.71¡Æ0.
4) The patients with blood group A were 35.72%, and blood type B, AB, & 0 were 4.04% 14.89% 10.63% in order.
5) The radiological findings of upper G-I series were seen filling defect in 56.75%, fill-ing defect with obstruction in 20.27%, filling defect with ulcer in 17.56% and ulcer in 5.4%.
6) On the location of the tumors, tumors were found 61.70% in antrum, 19.14% in. body and fundus, 5.13% in cardia to the lesser curvature and 9.56% in prepyloric and_ body to the greater curvature.
7) Incidence of-the gastric cancer with regional lymph node metastasis and celiac node metastasis were 78,72% and 27.65%. Distant metastasis found during operation was as; following; omentum 20.21%, liver 19.14%, trarsverse colon 17.01%, peritoneum 9.5700 and biliary system 6.3%.
8) 3 cases were diagnosed as early cancer by endoscopic biopsy. 1 of 3 cases with endoscopic biopsy was adenocarcinoma in situ.
9) Indication for surgical operation in 94 cases were proven diagnosis of gastric cancer 89.36%, pyloric obstruction 34.15%, gastric hemorrhage 9.54%, gastric perforation 7.44%, and clinically benign lesion 6.3%.
10) On the 94 patients operated gastric cancer, following surgical procedure were:;, performed: Tosal gastrectomy 2.12%, partial gastrectomy 51.05%, exploatory laparotomy and biopsy 21.27% and by-pass procedure 20.21%. The resectability was 53.17%.
11) Postoperative complications were infection 11 cases (44.00%), obstruction ;5 cases (20.00%), peritonitis 4 cases (16.00%), bleeding 3 cases (12.00%) and pneumonia 2 cases ($. 00%).
12) Operative mortality rate was 7.4%.
